Simon Forman (1552-1611), English astrologer, occultist and herbalist. After studying in Oxford and then Holland, he worked as a teacher in the 1570s and 1580s. He set up as a physician in London from 1583, moving towards astrological and occult practices from 1597. This led to him being banned from practising medicine, and he was imprisoned several times. He amassed a large collection of papers and manuscripts on a wide range of topics. Sixty-four volumes of his manuscripts are held at the Bodleian Library, Oxford, with others held in Plymouth Library. Artwork from 'Antiquarian Repertory' (1807).
